Eligibility: The page includes people over 60, EU Disability Card holders or valid Pink Form holders.
Application: Use the official page to contact the provider or reach the current application form. Ask about the assessment, waiting list and available service.
Documents: Application and the relevant disability card or Pink Form, where applicable.
Cost: The source describes labour exemptions for qualifying card holders and fees for other eligible older people. Materials are paid by the person. Confirm the job and charges.
