Provider: Active Ageing and Community Care
Eligibility: The page lists people over 60 or people under 60 holding a valid EU Disability Card, subject to the service’s checks.
Application: Use the official page to contact the provider or reach the current application form. Ask about the assessment, waiting list and available service.
Documents: Application including a GP-signed medical report.
Cost: A subsidised meal charge applies; check the current price before arranging deliveries.
Referral: Ask which referral route applies to the requested service.
