Practical planning guide
Ask for help with everyday activities
Turn a broad concern such as “I am struggling at home” into concrete questions for an assessment.
Practical next steps
- Choose two or three activities that matter most to you, such as washing, preparing food, using transport or meeting someone.
- Describe what makes each activity difficult, when help is needed and whether the difficulty changes from day to day.
- List the equipment, adaptations and informal help already in place, including what is not working.
- Ask which service fits the need: occupational therapy, equipment assessment, domestic help or skills support have different purposes and criteria.
- Agree goals in your own words and ask how your preferences will shape the plan.
- Confirm who will do what, any personal costs, waiting arrangements and when the plan will be reviewed.
- If a service cannot help, ask which alternative assessment or provider would be appropriate.
