Practical planning guide
Ask for practical support without a confirmed diagnosis
Prepare an enquiry based on your current difficulties while being clear about what is still under investigation.
Practical next steps
- Describe what you need help with now, such as getting to an appointment, shopping, communication, personal care, work or school.
- Explain which difficulties are observed, how often they occur and what help is currently available. State that investigations are ongoing if that is the case.
- Ask the provider whether it can assess current needs or requires particular medical evidence or confirmed eligibility.
- Ask what the referral must contain and who is authorised to complete it. Avoid paying for a report before confirming what is needed.
- Sapport Intake’s published criteria include a disability diagnosis, ages 0–60 and consent. If these do not fit, ask a GP or health-service social worker about other routes; a rare-disease label is not the same as disability-service eligibility.
- Request the next step, expected response arrangements and a contact for questions. Keep a short record of the answer.
- If you are refused, ask for the reason and any review or alternative route. Do not assume every scheme accepts undiagnosed applicants.
