Medicine-entitlement administration
Pharmacy of Your Choice and Schedule V administration
Eligible medicine entitlements can be collected through participating community pharmacies. A condition entitlement, an approved medicine and pharmacy registration are different administrative steps.

- Pharmacy of Your Choice Unit
- Eligibility
- A diagnosis alone does not establish entitlement. Ask the provider to assess the person’s needs and current programme criteria.
- Application
- Ask the treating clinician about the appropriate entitlement application and ask the chosen pharmacy what registration documents are required. Confirm approval before assuming an item is covered.
- Documents
- Ask about the entitlement document, valid prescription, identification and any medicine-specific approval. Do not send these to this website.
- Cost
- Coverage follows approved entitlement and formulary rules; it is not a promise that every medicine or product is free.
- Referral
- The relevant clinician completes medical entitlement requests; dispensing and approval rules remain with the official services.
- Funding
- This is not a promise of a payment, reimbursement, place or equipment allocation.
- Contact
- Use the contact details on the official page.
